Description:  

Objective: Enhance the health and well-being of Indian Americans in Lake County by providing accessible and comprehensive health services, aligned with the county’s health priorities. The program aims to promote early detection of health issues, educate participants on preventative care measures, and support mental health through counseling and workshops.

Description: The Community Health and Wellness Check-up Camps are organized biannually in collaboration with local hospitals, health departments, and community centers. These camps offer a range of essential health services including physical health check-ups, screenings for chronic diseases such as diabetes and hypertension, vaccinations, and health education sessions. Mental health support is integrated with counseling services provided by licensed professionals and workshops addressing stress management, mindfulness, and overall mental well-being. Participants also engage in interactive sessions on nutrition, physical fitness, and lifestyle modifications to promote healthier habits.

Eligibility: Indian Americans residing in Lake County

Estimated Participants: 250-300 per session

Duration: Two sessions per year, spaced 6 months apart

Expected Outcomes:

• Improved overall health outcomes through early detection and management of diseases.

• Enhanced awareness and understanding of mental health issues, leading to reduced stigma and improved mental well-being.

• Adoption of healthier lifestyle choices and behaviors among participants.

Program Governance:

• Evaluation and Reporting: Regular progress reports will be prepared for stakeholders, including a final comprehensive report summarizing the program's success, challenges, and recommendations for future initiatives.
